When purchased, our One-Year Eyewear Protection Plan covers all purchases of complete-pair frame and lens purchases or, Lens only and frame only purchases. The warranty begins on Day 1 and cost $29.99 upfront for the plan. Should a customer break or damage their eyewear during the first year of purchase, they simply pay the following co-pay to have the damaged part(s) replaced.
Given that a percentage of eyewear is delivered beyond day 1 (purchase date) we will always honor the dispensing date as the first day. The life of the protection plan is 1 year from the dispensed date and is not available past the 1 year.

This is a service contract (referred to hereinafter as the "Contract"). By purchasing it, you understand that it is a legal contract and acknowledge that you have had the opportunity to read the terms and conditions set forth herein. This is not a contract of insurance.
Obligor: Asurion Service Plans of Puerto Rico, Inc. (ASPPR) is the Provider/Obligor. ASPPR can be contacted at 875 North Michigan Avenue, Suite 3100, Chicago, IL 60611-1896, telephone 866-856-3882.
Instructions: You must keep the receipt for this product; it is an integral part of this Contract and you may be required to reference it to obtain service. This Contract, including the terms, conditions, limitations, exceptions and exclusions, the sales receipt containing the term of this Contract, constitute the entire agreement.
Definitions: Throughout this Contract the words (1) "we", "us", and "our" refer to the company obligated under this Contract, as referenced in the Provider/Obligor section; (2) "retailer" and "LensCrafters" refer to LensCrafters, Inc.; (3) "breakdown" refers to the failure of the product caused by: (a) defects in workmanship and/or materials, (b) normal wear and tear and (c) accidental damage from handling (ADH); (4) "accidental damage from handling" refers to the unintentional or accidental damage that occurs in the course of normal use or handling. (5) "product" or "eyewear" refers to the glasses or sunglasses that you purchased concurrently with and is covered by this Contract; (6) "you", "your", and "service contract holder" refers to the individual who purchased the product and this Contract; (7) "Service Fee" refers to the amount of money you are required to pay the retailer, as set forth in the Service Fee section of this Contract; (8) "authorized service center" refers to the center that performs the replacement or service of Your Product.
To Obtain Service: You must call ASPPR prior to having service; all repairs must be authorized in advance. If your product experiences a breakdown, return it to the retail location from which you purchased the product, or any other LensCrafters location if you are unable to return to your original location. If you have moved or are unable to return to the retailer, call ASPPR twenty-four (24) hours a day, seven (7) days a week, at 1-877-753-6727 (1-877-7LENSCRAFTERS) for instructions on obtaining service for your product. Please have your Contract handy and be prepared to tell us which product needs service and the nature of the problem. In some instances, we may authorize you to have your product repaired by a party that is outside of our servicer network and reimburse you for the repair. We will not reimburse you for work done by unauthorized servicers. Foreign language assistance is available for your convenience.
What is Covered: This Contract covers parts and labor costs to repair your product in the event your product experiences a breakdown as defined in this Contract. We will repair or replace the lens (lens service), frame (frame service), or both the lens and frame (lens and frame service) of the product, at our discretion, when required due to a breakdown. Non-original manufacturer's parts may be used for repair of the product if the manufacturer's parts are unavailable or more costly. You are responsible for delivery or the cost of delivery of your product to the authorized service center or a LensCrafters location for repair, per our instructions. If the covered product cannot be repaired, if the cost of the repair exceeds the original purchase price, or if parts are no longer available due to the age of the product or discontinuance by the manufacturer, we will, at our discretion, replace your product with a product of like kind and quality that performs to the factory specifications of the original product. There may be a Service Fee, see the Service Fee section below for more information.
Term of Coverage: Your term and coverage commences on your date of purchase and continues for a period of one (1) year, as indicated on your sales receipt.
Deductible: There is no deductible for this Contract. However, there may be a Service Fee. Please refer to the Service Fee section for more details.
Service Fee: You will be required to pay the retailer for the following services: (a) frame service = $15; (b) lens service =$15; (c) frame and lens service = $30.
WHAT IS NOT COVERED:
(1) INCIDENTAL OR CONSEQUENTIAL DAMAGES, DELAY IN RENDERING SERVICE UNDER THIS CONTRACT OR LOSS OF USE DURING THE PERIOD THAT THE PRODUCT IS AT A REPAIR CENTER OR OTHERWISE AWAITING PARTS; (2) ANY AND ALL PRE-EXISTING CONDITIONS THAT OCCUR PRIOR TO THE EFFECTIVE DATE OF THIS CONTRACT; (3) DAMAGE FROM INTRODUCTION OF FOREIGN OBJECTS INTO THE PRODUCT, OR UNAUTHORIZED PRODUCT MODIFICATIONS OR ALTERATIONS; (4) FAILURE TO FOLLOW THE MANUFACTURER'S CLEAN AND CARE INSTRUCTIONS; (5) LOSS OR DAMAGE DUE TO THIRD-PARTY ACTIONS (FIRE, COLLISION, VANDALISM, THEFT, ETC.); (6) LOSS OR DAMAGE DUE TO THE ELEMENTS OR ACTS OF GOD; (7) LOSS OR DAMAGE DUE TO WAR, INVASION OR ACT OF FOREIGN ENEMY, HOSTILITIES, CIVIL WAR, REBELLION, RIOT, STRIKE, LABOR DISTURBANCE OR CIVIL COMMOTION; (8) DAMAGE COVERED BY ANY OTHER WARRANTY OR SERVICE CONTRACT; (9) PREVENTATIVE MAINTENANCE; (10) DAMAGE WHICH IS NOT REPORTED WITHIN THIRTY (30) DAYS AFTER EXPIRATION OF THIS CONTRACT; (11) ANY LOSS OTHER THAN A COVERED BREAKDOWN OF THE PRODUCT; (12) DECORATIVE EMBELLISHMENTS AND/OR ACCESSORIES ATTACHED TO THE PRODUCT; (13) DAMAGE CAUSED BY ANIMALS AND INSECTS; (14) UNAUTHORIZED REPAIRS; (15) SERVICE OR REPLACEMENT OUTSIDE OF PUERTO RICO UNLESS EXPRESSLY AUTHORIZED BY ASPPR; (16) DAMAGE INCURRED DURING SHIPMENT OF THE PRODUCT FROM THE MANUFACTURER TO THE RETAILER; (17) PRODUCTS THAT ARE LOST OR STOLEN; (18) EYE EXAMS OR OTHER MEDICAL EXPENSES ASSOCIATED WITH OBTAINING REPLACEMENT EYEWEAR; (19) CLEANINGS, ADJUSTMENTS, AND FITTINGS; AND (20) REPAIR OR REPLACEMENT OF EYEWEAR DUE TO OUTGROWTH OF EYEWEAR, A CHANGE IN OPTICAL PRESCRIPTION, OR ANY OTHER MEDICAL REASON.
Transfer: This Contract is non-transferable.
Renewal: This Contract is not renewable.
Cancellation: The Contract shall be cancelled by us for fraud or material misrepresentation, including but not limited to commercial or rental use. Unauthorized repair or replacement of covered equipment shall result in the cancellation of the Contract by us. In the event of cancellation by us, written notice of cancellation shall be mailed to you not less than sixty (60) days before cancellation is effective. Such notice shall state the effective date fo the cancellation or termination and the reason therefor. The Contract can be cancelled by you at any time for any reason by mailing or delivering to us notice of cancellation: (a) within thirty (30) days of the receipt of the Contract, you shall receive a full refund of the price paid for the Contract provided no service has been performed, or (b) after thirty (30) days, you will receive a pro rata refund.
For a Contract that provides for coverage from the date of purchase of the product, if the Contract is cancelled before the expiration of the manufacturer's warranty and the pro-rated amount of premium earned exceeds sixty-five dollars ($65.00) multiplied by each year that the Contract has been in effect prior to the expiration of the manufacturer's warranty, the product of this amount shall be retained as the minimum premium. If the pro-rated amount of premium earned is less than the product, the pro-rated amount of premium earned shall be retained.
If we fail to pay any reimbursement or credit to you as required by this section within thirty (30) days following the date you mail the cancellation notice to us, you will be entitled to a monthly penalty equal to ten percent (10%) of the reimbursement or credit amount in addition to the reimbursement or credit amount.
No cancellation fee applies to this Contract.

Insurance Securing This Plan: Obligations of the Provider under the Contract are guaranteed under a reimbursement insurance policy. The Contract is secured by an insurance policy issued by The Continental Casualty Company. If the Provider fails to pay or provide service on a claim within sixty (60) days after proof of loss has been filed, you are entitled to make a claim directly against the insurance company. Please submit your claim in writing and include a copy of the Contract to: The Continental Casualty Company, 333 South Wabash Avenue, Chicago, IL 60604, telephone: 312-822-5000.
Non-Compliance of Duties: If any duties under the Contract are not performed, you may make a claim under the insurance policy as set forth above, or you may make a claim directly to the Provider.
